Why uric acid can look normal during a gout attack

It is one of the more disorienting experiences in gout. The joint is unmistakably inflamed, you get a blood test, and the uric acid comes back normal. The number that is supposed to explain everything says nothing is wrong.

This is common, it is documented, and it does not mean the diagnosis is wrong.

What happens during a flare

Serum urate frequently falls during an acute attack — sometimes into the reference range. Several things contribute: urate moves out of the blood as crystals form and as inflammation proceeds, and the body's response to acute inflammation appears to increase urate excretion. The net effect is that the moment you feel worst is one of the least reliable moments to measure.

The practical consequence is that guidelines generally suggest measuring serum urate once a flare has settled — typically some weeks afterwards — rather than treating a mid-attack reading as your baseline.

The reverse is also true, and worth saying because it cuts the other way: a high reading does not confirm gout either. Plenty of people have raised urate for years without ever developing symptoms. Hyperuricaemia is a risk factor, not a diagnosis, and diagnosis rests on the clinical picture — and definitively on identifying crystals in joint fluid.

Why a single reading is weak evidence generally

Even outside a flare, one measurement is a thin basis for anything. Serum urate moves with:

  • Hydration — dehydration concentrates it, fluid intake dilutes it
  • Recent meals and alcohol — a heavy meal or a night out shifts it short-term
  • Medication changes — starting or adjusting urate-lowering therapy moves it over weeks, and diuretics and low-dose aspirin push it up
  • Ordinary biological variation — the same person on two consecutive mornings will not produce identical numbers

None of this makes the test useless. It makes one test a poor proxy for a trend. What tells you something is the same measurement, repeated over months, in a consistent unit, with a record of what changed in between.

The three numbers people confuse

Reading your own results is easier once these are separated. All three are population-level figures, not statements about you:

The lab's reference range — roughly 208–428 µmol/L (3.5–7.2 mg/dL) for adult men and 155–357 µmol/L (2.6–6.0 mg/dL) for adult women, though every laboratory sets and prints its own. This describes the middle of a reference population. It is not a treatment goal.

The hyperuricaemia threshold — above 420 µmol/L (7.0 mg/dL) in the Chinese 2019 guideline, applied to adults of either sex. This is roughly where urate approaches the limit of its solubility in blood.

The treatment target — below 360 µmol/L (6.0 mg/dL) for people on urate-lowering therapy, and below 300 µmol/L (5.0 mg/dL) where there are tophi, frequent flares or joint damage. ACR 2020, EULAR 2016 and the Chinese 2019 guideline converge here.

Notice that the treatment target sits below the top of the normal range. Someone being treated for gout can be comfortably "normal" by lab standards and still above the number their doctor is aiming for. This is a frequent source of confusion in both directions.

What to track instead of chasing one number

If a single reading is unreliable and a flare-time reading is misleading, what is actually worth recording?

Readings with their dates, in one unit. The trend is the signal. Switching between µmol/L and mg/dL across years makes a history hard to read — pick one, or use something that stores one and displays either.

Whether you took the medication. Roughly two in five people with gout do not take urate-lowering therapy as prescribed, and adherence explains more variation in outcomes than most dietary questions. It is also the thing a doctor cannot verify and you cannot honestly reconstruct from memory three months later.

When flares happened, and what was around them. Dates, which joint, how bad, how long. Over a year this becomes the most informative thing you own about your own condition.

Whether the reading was taken during a flare. A note that takes two seconds and stops a misleading data point from quietly distorting a trend line later.

Bringing it to an appointment

An appointment is ten minutes and starts with questions you are expected to answer from memory: how have your levels been, are you taking it regularly, how many flares since last time. Those are all answerable precisely if something was writing them down.
